Cutting a hole at an angle through solid shape?

I'm trying to design an adapter to mate an automotive throttle body to an intake manifold.

It would use four holes at the corners of a 100x100x25mm thick block as pass throughs for the bolts to attatch it with. That's the easy part.

Now on the front of the block (the throttle body side) centered squarely is a 60mm hole. On the back side of the block (the intake manifold side), the same 60mm hole is off center. For simplicity's sake let's say it's 5mm north of dead center but retains the same diameter (60mm). I'd like the front hole cut at the correct angle to maintain the 60mm diameter from the front hole to the back hole.

How would I draw that shape? If I view the object from the side, the hole through the block would look like two parallel lines that slope upward 5mm from front to back.

I realized what I'm trying to say might be confusing. I drew it out in MS Paint (yea, I know). Hopefully this clears up what I'm shooting for. Left is front view, right is side view.
